Enhanced Care Management

Through Enhanced Care Management (ECM), each young person is connected with a dedicated care coordinator who partners with them to create a plan tailored to their unique goals and circumstances.
What makes ECM at Casa de Amparo unique is our commitment to continuity and trust. Rather than being passed from one provider to another, youth have one consistent care manager by their side—someone who checks in regularly, coordinates across systems, and ensures that care is responsive, trauma-informed, and youth-driven.

Who is eligible?
Services are provided at no cost to eligible youth through Medi-Cal, if:

You’re on Medi-Cal, under 21, and need extra support with health, housing, or life transitions, you may qualify

You’ve been in foster care (up to age 26)

Why Enroll in a Managed Care Plan (MCP?)
When you sign up for an MCP, you can access extra services at no cost to you that go beyond regular health care:

Help paying rent – through Community Supports, your rent can be covered for up to 6 months while you work toward stability.

Housing support – staff can help you search for housing, connect with landlords, and stay housed once you move in.

Personal care team – you’ll have a care manager who checks in, listens to your goals, and helps you navigate systems.

Health appointments made easier – support with scheduling, reminders, and even transportation to get you to appointments.

Mental health and wellness support – help connecting with therapy, counseling, or other services when you need them.

Basic needs connections – assistance with food, clothing, phone access, and other essentials to stay on your feet.

Education and job support – guidance to connect you with school programs, job training, or employment opportunities.

Advocacy – someone on your side to talk with doctors, social workers, or landlords so you don’t have to do it all alone.

24/7 access – many MCPs have hotlines and resources you can call anytime for support.
